Coordinate patient care activities to ensure safe, efficient hospital operations during assigned tours of duty.
Coordinate patient admissions, discharges, and intra- and inter-facility transfers; document patient disposition in shift reports.
Conduct nursing rounds to assess patient care needs, staffing levels, environmental conditions, and safety concerns.
Coordinate call-back and support services (e.g., Facilities, Biomedical, Laboratory, Radiology, OR staff, and medical staff).
Assist with emergency patient care using the nursing process and exercise sound clinical judgment.
Utilize Electronic Health Records (EHR) and Patient Classification Systems to document care coordination activities.
Ensure compliance with HIPAA, Privacy Act, IHS policies, and information technology security requirements.
Maintain professional conduct, cultural sensitivity, and clear communication in a multidisciplinary healthcare environment.
Rotating shifts including days, evenings, nights, weekends, and holidays.
Schedule and tour of duty assigned by the Patient Care Coordinator Supervisor Nurse or designee.
Minimum 36 months of nursing experience, including at least 2 years in a Tour Coordinator or patient care coordination role.
Experience with performance improvement initiatives, evidence-based practice, and data collection/analysis.
Current, unrestricted Registered Nurse (RN) license in a U.S. state or territory.
Basic Life Support (BLS) – required.
Advanced Cardiac Life Support (ACLS) – required at time of selection.
