Job Title: Level IV Respite Care Provider
location : Nationwide Opportunities — Family Residence, CYS Facility, or Other Approved Location
Job Type : Part-Time / As Needed
Responsibilities :
- Provide advanced one-on-one respite care for EFMP family members with complex needs.
- Perform all Level III provider duties.
- Assist with seizure activity support.
- Assist with tube feeding activities as authorized.
- Assist with tracheotomy suctioning as authorized.
- Monitor home medical equipment and report concerns.
- Follow the family’s service plan and document services provided.
- Maintain confidentiality, safety, and professional conduct.
Qualifications :
Must meet one of the following:
- Current Registered Nurse license; OR
- Current Licensed Practical Nurse license with at least 1 year of clinical experience; OR
- Bachelor’s Degree in a related field, such as Nursing or Special Education, with at least 2 years of documented experience providing complex care to individuals with special needs.
Additional Requirements:
- Favorable Tier 1 Background Investigation.
- National Sex Offender Registry Check.
- State and local child abuse registry checks.
- Current CPR certification with hands-on, in-person skills assessment.
- Online-only CPR certification is not acceptable.
- Ability to safely provide care to individuals with complex medical or special needs.
