Provide evaluation and treatment for patients with diverse conditions (neurological, musculoskeletal, post-surgical, prenatal/postpartum, chronic pain, etc.).
Develop individualized treatment plans, set goals, and involve patients and families in care planning.
Deliver physical therapy interventions and adjust treatment regimens through periodic evaluations.
Collaborate with referring physicians and participate in the Patient Centered Medical Home (PCMH) model of care.
Graduate of an accredited Physical Therapy program.
Current, unrestricted license to practice Physical Therapy in at least one U.S. state or territory.
Minimum of 1 year of recent experience (within the past 36 months) as a Physical Therapist.
Current Basic Life Support (BLS) certification; ACLS preferred.
Experience with Electronic Health Records (EHR).
