Shall identifies, assesses, and coordinates client cases; and anticipates their needs related to illness, hospitalization, finances, education, and home/community service.
Serves as a liaison between military medical and behavioral health providers and community agencies.
Refer to LCSW’s for higher level assessment, referral, or support.
Be able to perform case management functions such as gathering medical documentation, getting releases of information signed by service members, and seeking out resources in the community.
Be able to present for the BH department at events, such as the yellow ribbon events.
Provide early intervention and assessment upon identification and onset of emotional; and behavioral concerns to restore and improve service member functioning.
Perform Initial and follow-up SM screening evaluations, utilizing information from clinical interviews, non-psychometric tests, and collateral sources, as appropriate.
Qualifications:
Shall have familiarity with behavioral health assessment tools: e.g., when screening service members for PTSD contractor shall use the PLC-M tool.
Education/Certification:
Shall have graduated with a Bachelor of Social Work.
