Job Title:  Wellness Coach

Duties:

  • The Wellness Coach will work with individuals on behavioral changes in their recovery related to nutrition, stress coping, sleep, mind-body balance, and balancing wellness practices in recovery.
  • The Wellness Coach will provide wellness support through group therapy, individual therapy, couples/family therapy, and family education, effectively utilizing evidence-based practices to meet the needs of each individual’s individualized recovery plan.
  • Develops and reviews individualized wellness service plans with persons served to monitor individuals' progress toward individualized treatment goals.
  • Completes assessments of the people served.
  • Attends and participates in case staffing and treatment team meetings.
  • Works with interdisciplinary team in reviewing cases and developing individual service plans, including referrals to case management, advocacy groups, skills training groups, social rehabilitation, and external treatment providers.
  • Provides clinical consultation to referral sources, community stakeholders, and members of individuals’ support system to help support individuals in their recovery process.

 

Education & Qualifications:

  • Education – bachelor’s degree in Behavior Healthcare field
  • Licensure/Certification – Job related license (LCSW, LPC, LMFT, MFT, LMSW, LAPC, MAC, CACII, CADC, CAC)
  • Experience – Minimum (3) year of human services, social work and /or clinical experience with addictive diseases population
  • Other – Criminal background check clearance, official academic transcripts, valid driver’s license/ID, drug screen, TB test.
